Class Members who acquired CV securities on the Toronto Stock Exchange ("TSX") during the period December 11, 2006 to March 23, 2007 and who held some or all of those securities on March 26, 2007 are eligible for compensation and should file a Claim using the secure Online Claim System.

• Registering a Claim to participate in the Settlement.
• Communicating with the Administrator with respect to an Online Claim.
• Editing and certifying an Online Claim.
• Printing a completed Online Claim.
• Uploading supporting documents to attach to a Claim.
• Preparing a Fax Cover Sheet to fax documents to attach to a Claim.
• Checking the status of a completed Claim.

• To complete the Claim Form online, you will need details of the number of CV Shares:
  • held on December 10, 2006
  • purchased, acquired, sold or disposed of in the period, December 11, 2006 to March 23, 2007
  • sold or disposed of in the period, March 24, 2007 to March 26, 2007, and
  • sold or disposed of after March 26, 2007

• The Administrator must receive the completed and signed Claim Form and all required Supporting Documentation, by the January 6, 2011 Claims Bar Deadline.

The Court has approved a December 2011 payout of claims in this action. Cheques have been mailed.
